Default surging

I have a 2000 Sonoma, I bought new in September 1999. It currently has 165,000 miles. No real issues in twenty years. It has the 4.6L engine with automatic transmission. Just lately the engine stated surging about 100 rpm between 45-50 mph. The rest of the time it runs like a clock. The truck is very well maintained and regularly serviced and completely stock. Looking for some help!
