1986 jimmy speedometer problem

I have been trying to get my spe3edometer operational. when i first got the truck the driven gear was stripped and would not grab the cable to turn it. i replaced the driven gear and it worked for a while then one day stopped.

i looked to see what happened, the cable and adapter were hanging and the lock plate was twisted. the adapter actually came apart. i then took the lockplate off the driven gear out, looked for signs of damage and there was none. i assumed maybe during my assembly process the first time i failed to tighten the bolt enough.

so i threw it all together, and it work for the next three days or so. i dont drive it much. this time i let my brother drive it when it happened. everything is still assembled. i can only assume the driven gear is stripped out again.

Does anyone have an idea on where to go from here? thanks for any help you can give.
