1991 Sonoma 4.3 rough idle

My 1991 runs very well while driving. The problem is that it idles very rough and frequently dies at stop signs and such especially with the A/C on. If I hold down on the gas pedal to increase the idle just slightly, it runs fine. I end up driving with 2 feet. I've changed plugs, checked all vacuum lines, cleaned AIC, cleaned rotor & dist cap. That helped some but still have the problem.
