Start with replacing the fuel filter and cleaning the EGR valve. The EGR valve in your year of 4.3 vortec are known to carbon up bad and cause what your have described. The other thing also that can cause this is a bad fuel injector system and your going to have to address that, start with the simple and replace the Fuel filter and clean the EGR valve and throttle body with throttle body cleaner. You may want to use Carburetor cleaner on the EGR valve since it's stronger then the throttle body cleaner, but don't use the carburetor cleaner on the Throttle body. The carburetor cleaner is to strong for the gaskets used on the throttle body. and will destroy it.

Make sure all your vacuum lines are good and not damaged or there is any vacuum leaks. Good luck and keep us posted.
