1999 K2500 throttle issues ?

I am having some issues with my 1999 Holden Suburban and need to know if anyone out there has experience or ideas.
The original issue started a few weekes ago when the engine reved high while driving for no reason. When it came back down to normal after a few seconds the engine stopped. The reving problem is getting worse, and now it occasionally does it on starting. The engine fires with a cloud of black smoke, rev's high then stops.


I have replaced all the earths and battery cables. I thought it had fixed it as it starts better and runs better on idle, however when it gets put into gear (its an auto) it starts the intermitent reving again.
Its the K2500 6.5L TD Aussie version imported ,converted and badged as a Holden

Any ideas would be appreciated
